Full Arch Dental Implant Contest Questionnaire

Contest Requirements

You must live in the U.S., speak English and are between the ages of 35 to 65. You must be experiencing some level of difficulty with eating with several missing or decayed teeth or dentures. You must have a strong desire to have dental implant supported teeth. You must be financially incapable of paying $20,000 - $60,000 for a dental implant procedure. You must be able of travel to a different city or state for treatment, if necessary. If you win, you must be willing to record a video interview before and after your dental implant treatment and willing to allow PayingForDentalImplants.com to publicly share your experience in an effort to inspire others. If for any reason either of these requirements can’t be met by you, we kindly ask that you would not enter this contest.

· Tell us about yourself (where are you from, what do you do for a living, disabled, vet, etc.)

· How did you end up with failing teeth or dentures?

· What problems are you dealing with as a consequence of living with your current oral condition?

· How long have you been dealing with this situation?

· What would it mean for you to replace your bad teeth or dentures with dental implants?
